Youth Athletic Camp Liability Waiver

I, [Guardian/Parent Name], on behalf of myself and my child, [Child’s Full Name], hereby acknowledge and agree to the terms of this liability waiver before participating in the Be Somebody LLC Youth Athletic Camp. In consideration for my child’s participation in the camp, I understand and accept the potential risks associated with physical activity and related activities. I acknowledge that Be Somebody LLC and its staff will take reasonable measures to ensure the safety and well-being of participants, but unforeseen circumstances and accidents can occur.

1. Assumption of Risk:
I understand that participation in youth athletic activities, including but not limited to weightlifting, cardio exercises, and other fitness activities, carries inherent risks, including the risk of injury. I acknowledge that my child is voluntarily participating in these activities with full knowledge of these risks.

2. Release of Liability:
I, on my behalf and on behalf of my child, hereby release and discharge Be Somebody LLC, its owners, instructors, employees, and affiliates from any liability for any injuries, damages, claims, or losses that may occur during or as a result of my child’s participation in the camp. This includes any claims arising from the negligence of Be Somebody LLC or its staff.

3. Medical Authorization:
I authorize Be Somebody LLC to seek and obtain necessary medical attention for my child in the event of an injury, accident, or illness during the camp. I understand that I will be responsible for any medical expenses incurred.

4. Photo/Video Release:
I grant Be Somebody LLC permission to use photos and videos of my child for promotional and educational purposes, including but not limited to the website, social media, and marketing materials.

5. Code of Conduct:
I understand that my child is expected to follow all camp rules and guidelines set by Be Somebody LLC. Failure to do so may result in their dismissal from the camp without a refund.

6. Governing Law:
This waiver shall be governed by and construed in accordance with the laws of the state of [Your State].
